Marriage

Apply to the Health Insurance Society if the name of the insured person or a dependent has changed or you want to add a new family member as a dependent due to marriage or other reason.

Change of name

To apply, if you are a Takeda Pharmaceutical Company Limited employee,

  • provide the necessary information on the following document,
  • attach your health insurance card,
  • attach an interoffice mail return envelope (size A4, with the destination address filled out),
  • and submit to the Health Insurance Society.

To apply if you are not a Takeda Pharmaceutical Company Limited employee,

  • provide the necessary information on the following document,
  • attach your health insurance card,
  • and submit to the section at your company responsible for health insurance administration.
Required documents:
  • Notice of Change to Registered Health Insurance Information
Health insurance card
Deadline: Immediately after the name change
Applies to: Insured persons and dependents whose names have changed

Notes:
  • ** In principle, complete the procedures above for the Health Insurance Society within five days if the name of an insured person or dependent has changed due to marriage, divorce, or other reason or you find an error in the registered data, such as the date of birth or name.

Adding a family member

A family member must be certified by the Health Insurance Society to become a dependent.
