Influenza vaccination subsidies Description FAQ Influenza vaccination subsidies The Health Insurance Society will pay the cost (including tax) of one influenza vaccination for each insured person or dependent when obtained through an employer’s group vaccination program or by visiting the hospital or other facility on your own (individual vaccination). The Society will pay the full cost of two vaccinations for dependents of less than 13 years of age. Eligible persons Insured persons and dependents Vaccination period October 1–January 31 Method [Insured person] Take advantage of any group vaccinations provided by your employer, if offered.If your employer does not provide group vaccinations, visit a hospital or other facility on your own (individual vaccination). You cannot get both the group vaccinations provided by your employer and individual vaccinations. [Dependents, Voluntarily and Continuously Insured Persons] Go to a hospital or other facility on your own (individual vaccination).Note: Vaccinations conducted by municipal governments also are included in individual vaccinations. Claiming and payment of vaccination costs ◎No procedures are required if you were vaccinated through your employer’s group vaccination program. ◎If you were vaccinated through individual vaccination, submit a claim as outlined below. If a dependent of less than 13 years of age has been vaccinated twice, submit a claim for the total cost of both vaccinations after the second vaccination is complete. Age for these purposes is the age as of the time of vaccination. When being vaccinated through individual vaccination, pay the cost of the vaccination at the hospital or other facility and be sure to obtain a receipt. «Notes on receipts» The receipt should be addressed to the person vaccinated (have one receipt prepared per person). The receipt must show the fact that the vaccination was for influenza. Costs claimed for individual vaccinations are generally paid in the month following the receipt of the application form. For Voluntarily and Continuously Insured Persons, payments will be made by direct deposit into their individual bank accounts. However, the timing of payments may vary with the payroll systems of individual companies.Note: Employers will notify employees via company bulletins or by other means of program details, including availability of group vaccinations.
